United Concordia dental plans are underwritten by Florida Combined Life which both Dr. Talbert and Dr. Barnes participate in. Please note that United Concordia Military coverage is accepted by Dr. Talbert only.

Please be advised that while we may not be in-network providers for your specific insurance plan, this does not necessarily mean your insurance will not provide coverage. it simply means we are considered out-of-network. We strongly recommend that you verify your insurance benefits and coverage with your provider prior to your appointment, as it is the patients responsibility to ensure services are covered and accepted at our office. As courtesy, we are happy to file claims for all insurance plans with the exception of DMO, HMO and savings plans.
